What is the difference between MODEL and PATTERN?

I'm trying to figure out why we are using model and pattern.

Model example: Asynchronous Programming Model (APM)

Pattern example: Task-based Asynchronous Pattern (TAP)), Event-based Asynchronous Pattern (EAP)
